Austin Diabetes Services
Diabetes is an ongoing disorder marked by elevated levels of glucose (hyperglycemia)
resulting from metabolic dysfunctions that inhibit adequate insulin production in
individuals (Ernawati et al., 2021b). Social, cultural, and health literacy elements are the
main determinants of people capacity to prevent positive health outcomes related to
diabetes and their capacity to self-manage. Diabetes Self-Management Education is an
evidence-supported initiative designed to aid people who have diabetes, attain an
improved quality of life. It follows accredited and verified procedures that guarantee the
provision of high-quality education.
Our Mission
Our mission is to empower individuals living with diabetes through evidence-based education,
ongoing support, and a comprehensive approach that recognizes and addresses the Social
Determinants of Health (SDOH) impacting their ability to care for themselves.
We strive to reduce health disparities, improve glycemic outcomes, strengthen self-efficacy, and
promote physical, emotional, and social well-being.
Through culturally competent education, community resource navigation, and personalized
support, our mission is to transform the diabetes experience so every individual can live with
dignity, confidence, and full ownership of their health.
Nonprofit status
Ve Valance wellness institute is a non-profit corporation and shall operate
exclusively for educational and charitable purposes within the meaning of
Section 501(c)(3) of the Internal Revenue Code, or the corresponding section of
any future Federal tax code.
DSME
Diabetes Self-Management Education (DSME) gives individuals the knowledge to
control their condition and improves health outcomes like lower A1C, heart disease and
kidney failure. Yet undeserved population DO NOT have access to these programs due
to lack of medical insurance coverage. Your donation supports Austin Diabetes
Services to provide the education and A1C testing to these communities!
Health Disparities and SDOH
Patients in our target population face significant social and structural barriers:

Stress, depression, or lack of social support
These barriers contribute directly to poor glycemic outcomes and preventable complications.
Gap in Services
Although diabetes care is available, structured DSMES integrating SDOH is largely absent.
Patients are often prescribed medications without education or support to implement lifestyle
changes—creating a gap this program aims to fill.
